Open the Health app and navigate to Health Records

Sign in with your patient portal credentials

The iPhone Health app has a feature called Health Records that pulls your medical information — lab results, medications, immunizations, allergies, conditions, and visit notes — directly from participating hospitals and doctor's offices into a private, secure location on your phone.
Once connected, your health data appears in the Health app under "Health Records." You can see your blood test results, vaccination history, active medications, allergies on file, and a timeline of your medical visits.
The data syncs automatically when your providers update it. You always have your up-to-date medical history with you — useful for emergency situations, new provider visits, or simply keeping track of your own health.
Your health records stay on your iPhone and are protected by your Face ID or Touch ID. Apple does not see your medical information.
This feature works with hospitals and clinics that use a compatible electronic health record system. Over 1,000 health systems participate including many major hospital networks, Kaiser Permanente, and VA health facilities. You can check if your provider supports it during setup.
Requirements: iPhone with iOS 11.3 or later (iOS 16 or later recommended for the best experience).
